As a Case Manager, Housing (Site Based), you'll partner with residents who have experienced long-term or chronic homelessness and support them as they build stability, strengthen independence, and work toward their personal goals. Through relationship-based case management, you'll help residents navigate challenges, connect to resources, maintain housing, and create pathways toward a brighter future.

What You'll Do
- Build trusting, person-centered relationships with residents and maintain an active caseload
- Complete assessments and partner with residents to develop individualized housing and wellness goals
- Support residents in accessing healthcare, mental health services, benefits, employment resources, and community supports
- Help residents maintain housing stability through proactive engagement, advocacy, and problem-solving
- Coordinate services with landlords, service providers, community partners, and internal teams
- Support residents through crisis situations, transitions, and barriers that may impact housing stability
- Promote tenant education, independent living skills, and housing success
- Maintain timely and accurate documentation, assessments, and service plans
- Collaborate with colleagues across programs to support positive participant outcomes
- Promote Housing First, Trauma-Informed Care, Motivational Interviewing, and person-centered practices throughout all services
