Case Manager Supportive Housing

At Wyandot Behavioral Health Network (Wyandot BHN), we believe recovery is possible and that every individual deserves compassionate, person-centered support. We're seeking a dedicated Case Manager to join our community-based team serving adults living with severe and persistent mental illness.
In this role, you'll build meaningful relationships with individuals and families, coordinate services across behavioral health and medical providers, connect clients to essential community resources, and advocate for the support they need to thrive. What You'll Do
As an Case Manager, you will:
- Develop trusting, recovery-focused relationships with individuals and their families.
- Conduct strengths-based assessments and collaborate on person-centered treatment plans that support each individual's goals.
- Coordinate care with behavioral health providers, primary care, hospitals, community organizations, and family members.
- Connect clients to housing, healthcare, financial assistance, employment, transportation, and other community resources.
- Advocate for clients with healthcare providers, courts, schools, government agencies, and social service organizations.
- Support individuals during periods of crisis by helping coordinate appropriate interventions and services.
- Monitor progress, adjust service plans as needed, and ensure clients receive the highest quality of care.
- Complete timely, accurate documentation that meets Medicaid, agency, and regulatory standards.
- Participate in team meetings, clinical supervision, ongoing training, and professional development.
- Travel throughout the community to meet clients where they are and, when appropriate, transport clients to appointments and services.
