Case Manager

The Center at Northridge is recruiting for a Case Manager to assist with patient placement referrals, assessments, care plans, maintaining patient records, audits and other duties as assigned.

This is a full time position eligible for medical, dental, vision and supplemental insurances, paid time off and 401(k) with match.

Job Type: Full-time

Salary: $60,000.00 to $75,000.00per year
