Intake and Eligibility Manager

The Intake and Eligibility Manager will provide for all functions of the departmental responsibilities and processes including as defined by He-M 505 including: receive referrals; assist individuals and their families/legal representatives in completing an application for eligibility in accordance with all applicable State laws and regulations; process applications for eligibility determination. The I/E Manager will be responsible for processing such applications through comprehensive file review and/or assessment to determine eligibility under state regulations. The Manager will oversee Transfers to and from Region 7 and will perform/facilitate standardized functional evaluation when needed. The Intake and Eligibility Manager will be responsible for data collection and tracking related to referrals, intakes, eligible and ineligible determinations, and other metrics as assigned.


Pay: $25-$30/hr.

Hours: 8am-4pm or 8:30am-4:30pm


Essential Responsibilities:

· Providing office presence and availability during business hours to receive and respond to referrals by potential applicants and external stakeholders, tracking relevant data

· Assist applicants/families and/or client representatives in making written application for eligibility determination in accordance with He-M 503.04 and/or He-M 522.03(a), and in accordance with Area Agency Standards and Policies

· Assist and/or guide individual/family/guardian in obtaining pertinent records (evaluations, assessments) and other relevant documentation that support and substantiate the individual’s application, and/or which allows the Area Agency to determine eligibility.

· Review each application for services and relevant records, being able to distinguish presence if necessary data and/or provide list of missing information.

· Ensure that the Area Agency adheres to eligibility determination timelines stated in He-M 503.05(d) and/or He-M 522.06(a), apply for extensions when appropriate, and document actions and timelines accordingly.

· Arrange for and coordinate comprehensive screening and evaluation of each client determined to be eligible in accordance with He-M 503.05 and/or He-M 522.06(a); this may include facilitating standardized interviews

· Meet with individual/family/guardian to determine services needed and approximate time frame of when services will be needed (i.e., Case Management, Family Support, Day Program, etc.)

· Notify applicant/representative of eligibility findings in writing (HeM 503.05 (h) and/or (HeM 522.06(b)

· Remain informed of appeals process and represent the Agency at an appeal when individual/guardian files an appeal; meet with Agency’s legal counsel, gather additional documentation, prepare and file documentation with Administrative Appeals Unit, and attend hearings.

· Demonstrate general working knowledge of steps for Medicaid applications and Probate Court guardianship petitions; have a general understanding of legal roles defined therein.

· Refer clients to all appropriate internal or external relevant community resources

· Ensure that client’s needs and rights during referral/eligibility process are considered and documented

· Understand initial service planning steps such as to refer eligible clients with families to Family Support department to determine need for respite services and/or other services

· Maintain documentation, data entry, and documentation storage processes defined by the Agency.

· Oversee completion of transfer process between regions: review documentation, meet with identified parties, gather additional documentation as needed

· Other duties and responsibilities as assigned by Supervisor
