School Based Community Case Manager

We are seeking a driven, adaptable, and compassionate professional to serve as a School-Based Case Manager within our Opportunity Academy, an alternative school setting supporting 7th-12th grade students with complex behavioral, emotional, and environmental needs.

This role is designed for a true go-getter, someone who takes initiative, thrives in fast-paced and unpredictable environments, and remains steady and solutions-focused during crisis situations. The ideal candidate brings a trauma-informed approach to every interaction and is passionate about helping vulnerable youth succeed both in and out of the classroom.

What You’ll Do

Student Support & Case Management

• Provide direct case management services to students identified with behavioral health needs.

• Develop, implement, and monitor individualized service plans in collaboration with students, families, and school staff.

• Support students in building coping skills, emotional regulation, problem-solving, and social skills.

• Maintain consistent contact with students to ensure engagement and progress toward goals.

Crisis Response & Intervention

• Respond to behavioral and emotional crises within the school setting using de-escalation and trauma-informed strategies.

• Provide immediate support and stabilization during high-risk situations.

• Collaborate with school staff and clinical teams to ensure student safety and appropriate follow-up care.
