Case Manager

McAlister Institute is seeking a compassionate and dedicated Case Manager to join our team. This individual will report directly to the Program Manager and assist clients in accessing vital services such as medical, educational, social, and vocational resources. Focus will be on coordinating Substance Use Disorder (SUD) care, integrating with primary care for clients with chronic substance use disorders or co-occurring conditions, and supporting interactions with the criminal justice system as needed.

Key Responsibilities:
- Uphold and promote the mission, philosophy, values, and direction of McAlister Institute.
- Develop and manage individualized case management service plans, ensuring clients have the support and resources they need to succeed.
- Perform comprehensive assessments and periodic reassessments to determine the continuation of case management services and revise treatment plans as necessary.
- Assist clients in applying for and maintaining health and public benefits (e.g., Medi-Cal, Healthy San Diego, Housing).
- Provide recovery support through outreach, life skills workshops, socialization, and group activities to support treatment completion.
- Act as an advocate for clients across multiple systems, ensuring timely and appropriate access to physical and mental health care, housing, and other essential services.
- Maintain accurate records and documentation, adhering to all DMC standards and agency policies.
- Coordinate transitions in care across ASAM levels, including making referrals and working with treatment agencies to ensure smooth transitions.
- Develop and maintain relationships with community partners to ensure clients have access to all available resources and services.
- Facilitate transportation for clients to medical appointments and treatment sessions when necessary.
